PandaSama's Realistic Childbirth mod just got its most significant family gameplay expansion yet: a fully scripted home birth workflow that replaces the mod's previously hospital-centric delivery system with an in-home alternative built around object placement, midwife coordination, and a specific interaction chain players must follow exactly.
The workflow begins in the second trimester, when "Call Obstetrician" becomes a decision point rather than a straight appointment trigger. Choosing home delivery at that stage sets the entire sequence in motion. From there, two physical setups are available: a birthing pillow placed on a double bed for a bed birth, or a birthing pool for a water birth. Both require deliberate lot planning. PandaSama's documentation flags that the bassinet spawned at birth needs clear floor space around the delivery object; if that space is blocked by furniture or walls, the bassinet can appear in an unreachable location and the baby becomes inaccessible.
Once labour begins, the interaction to use is "Start Home Birth," not the vanilla game's "Have Baby" option. The two systems are incompatible, and using the base game interaction mid-mod flow breaks the animation sequence and skips the midwife checks the mod is built around. PandaSama's own documentation makes this explicit.
Midwives are optional for bed births but mandatory for water births, and they can be called during active labour rather than scheduled in advance. The in-mod service fee runs at $120 per hour of attendance. A Sim with the doctor career can also perform some clinical checks if present in the household during delivery, adding a secondary layer for players who have built out medical career storylines. Inviting a birth companion ahead of labour, rather than scrambling mid-sequence, is another practical note flagged in walkthroughs of the update.
This home birth layer lands on top of a system that already includes pregnancy complications, NICU mechanics, and postpartum recovery options from earlier PandaSama updates.
