A contract between Blue Shield of California and Community Health System lapsed Feb. 1, 2026, removing Community Medical Centers from Blue Shield's in-network provider list and forcing patients to find new in-network care. Blue Shield said it made offers with "fair and reasonable rate increases" and proposed tying some payments to performance, adding that CMC was "not willing to tie any portion of their pay increases to performance." Blue Shield also claimed Community Health System requested to terminate the existing contract with the intent to renegotiate. Community Medical Centers countered that Blue Shield "elected not to grant another extension and let it expire January 31, 2026" and said it is "actively working with Blue Shield to reach an equitable agreement as soon as possible so we can continue providing quality healthcare to those who rely on us."

The immediate consequences are practical and financial. Fresno City Manager Georgeanne White warned, "The city employees are frankly stuck in the middle and as a result of the contract expiring, these city employees are going to be forced to pay out of network premiums, which is going to cost them more money." Patients can also face interruptions in specialty care and potential surprise bills. Blue Shield has advised affected members to contact their doctors to discuss in-network alternatives, including Saint Agnes Medical Center and Valley Children’s Hospital. California’s Right of Continued Care law may allow some patients to keep their specialist for a limited time, but protections and timelines were not specified.

This dispute sits alongside a separate deal that restored coverage for over 6,000 Fresno Unified School District retirees under a different insurer, highlighting how local coverage is fragmented by employer contracts and insurer networks. Negotiations between Blue Shield and Community Health System are ongoing, but neither side has offered a timeline to restore in-network status.
