Teresa Petitti, 39, of Port Jervis was pronounced dead Monday after her 2024 Honda left the westbound lanes of State Route 17 in Mamakating, struck a guide wire on the northern shoulder, crossed two lanes of traffic and slammed into the center median guide rail. New York State Police said the crash occurred at approximately 10:40 a.m. in the Sullivan County town.

Troopers from the State Police Wurtsboro barracks were called to the scene of the single-vehicle collision. Before they arrived, passing motorists pulled Petitti from the wrecked Honda. Troopers then performed life-saving measures at the scene until emergency medical services arrived.

Petitti was transported to Garnet Health Medical Center, where she was later pronounced dead.

A preliminary investigation determined the 2024 Honda was traveling west on Route 17 when, for unknown reasons, the vehicle drifted onto the northern shoulder and struck a guide wire. The car then re-entered the roadway, traveled across two lanes of traffic and struck the center median guide rail. State Police have not identified a cause for why the vehicle left the road.

The investigation remains ongoing. Anyone who may have witnessed the crash is asked to contact State Police in Middletown at 845-344-5300 and reference case number NY2600395823.
