Don Etienne Memorial Golf Scramble will tee off Oct. 14 at Christmas Lake Golf Course in Santa Claus to raise funds for Perry County Memorial Hospital; register by Sept. 27.

The Perry County Memorial Hospital Foundation will stage the Don Etienne Memorial Golf Scramble at Christmas Lake Golf Course in Santa Claus on Oct. 14 to raise money for hospital programs, equipment upgrades, and patient services, with a registration deadline of Sept. 27. Registration opens at 8:00 a.m. the day of the event, with tee off set for 9:00 a.m., lunch at 1:00 p.m., and awards at 2:00 p.m.
Teams can enter as a four-person group for $400 or as individual golfers for $100; the event flyer instructs teams to register online through the hospital’s foundation registration page or to complete the paper form on the back of the flyer by Sept. 27. The team registration form asks for company name as it should appear in materials, contact name, address, city/state/ZIP, phone number and e-mail, and includes a checkbox area for sponsor and entry selections.
Sponsorship opportunities range from an Exclusive Title Sponsor at $5,000 to Gold Sponsor at $750. The flyer lists the Exclusive Title Sponsor as $5,000 (one available) and includes a four-person team, two yards of string to be used on greens, four unlimited drink wristbands, exclusive title sponsor naming rights, logo signage at the clubhouse and at every hole, title sponsor recognition on all promotions, and a promotional item in golfer swag bags. Cart Sponsor is $2,000 (two available) and includes logo signage on all carts, a four-person team, one yard of string for use on greens, program and social media listing, tee sponsor sign on four holes, and three drink tickets per person. Platinum Sponsor is $1,250 (four-person team, one yard of string, program and social media listing, tee sponsor sign on four holes, two drink tickets per person), and Gold Sponsor is $750 (four-person team and tee sponsor sign on two holes). Contest Sponsor is $200 and provides sponsor signs at contest holes; the flyer specifies Closest to the Pin on Hole #13 and Longest Drive on Hole #16. Tee Sponsor is $125 for a tee sponsor sign at a tee box.
Player add-ons are available but restricted: mulligans are offered as 4 for $20 and toss as 4 for $20, with the flyer noting “Limit 2pp/8 per team” and explicitly stating “ADD-ONS (cannot be used on hole 13 or 16).” The contest-hole rules on the flyer pair with the add-on restriction by naming hole #13 for Closest to the Pin and hole #16 for Longest Drive, and the flyer repeats those contest and fee blocks in multiple sections.

The flyer’s beneficiary line reads “ALL Proceeds Benefit” but is truncated on the printed material; the foundation’s summary language describes proceeds going to “hospital programs, equipment upgrades, and patient services.” The full sponsorship table on the flyer shows Exclusive Title Sponsor at $5,000; the registration-form fragment also contained an inconsistent truncated line that listed “Exclusive Title Sponsor, [...] $750,” which appears to be a layout error on the flyer and is reconciled by the detailed $5,000 listing and the stated availability of one Exclusive Title Sponsor.
With the Exclusive Title Sponsor slot limited to one and only two Cart Sponsor slots listed, organizers encourage early registration ahead of the Sept. 27 deadline to secure team entries and sponsor packages for the Oct. 14 scramble.
