Revive & Thrive opens downtown Perham clinic offering neuro-rehabilitation services
Revive & Thrive opened a downtown Perham clinic offering neuro-rehabilitation and in-home therapy, bringing specialized services closer to Otter Tail County residents.

Revive & Thrive Physical & Occupational Therapy opened a downtown Perham clinic at 139 1st Ave S, adding local access to specialized neuro-rehabilitation and in-home therapy that previously sent some patients to larger cities. Owners Lisa Yonker, a Doctor of Physical Therapy, and Tina McDonald, a registered and licensed occupational therapist, said the practice aims to meet demand in the lakes area for stroke, brain injury, concussion, spinal cord injury and degenerative disease care.
The clinic provides both in-clinic appointments and home visits. “In addition to offering services at the downtown location, 139 1st Ave South, Revive & Thrive provides assistance at a client’s home. And, clients do not need to be considered ‘homebound’ to receive those services, McDonald noted.” Business hours are Monday through Friday, 9 a.m. to 4 p.m. Appointments can be made by calling Yonker at 218-332-4096 or emailing lisayonker@reviveandthrivetherapies.com.
Yonker framed the opening as a way to keep care local. “We really hope that we can bring those types of skills to the lakes area to prevent patients from having to go to Fargo for it.” She also emphasized routine geriatric supports: “We even help with general geriatric stuff like fall prevention and balance,” which are common needs for older residents and can reduce emergency visits and related costs.
Yonker’s background includes graduating from the University of North Dakota’s physical therapy program in 2015 and clinical experience treating spinal cord injury patients, a path that shaped her neuro-rehabilitation focus. Perham Focus reported that Yonker and McDonald bring “decades of experience” and “specialized training that is only offered in larger cities,” positioning the clinic as a higher-tier local option for neurologic and degenerative conditions such as Parkinson’s disease, multiple sclerosis, muscular dystrophy and ALS.

From an economic perspective, adding specialized outpatient services in Perham can keep health-care spending in-county, reduce travel expenses for families, and modestly expand the local health-care labor market. For residents of Otter Tail County and the surrounding lakes area, that means fewer long drives for appointments and faster access to follow-up care that can improve outcomes and potentially lower long-run care costs.
Readers should note a business-directory discrepancy: the Perham Area Chamber of Commerce lists Revive & Thrive at 185 East Main with a chamber contact number, 218-346-7710, but the clinic’s downtown address and direct appointment number above align with the open-house and Perham Focus reporting. The clinic’s website carries the tagline “Heal, Strengthen, Flourish” and shows a copyright notice dated 2025.
What this means locally is practical: Perham residents with neurologic or balance issues now have a nearby clinic that offers both clinic-based and in-home care. Call 218-332-4096 or email lisayonker@reviveandthrivetherapies.com to confirm services, hours, insurance acceptance and program details.
