For certain boards such as the ‘Amat Kuhwely, LLC Board of Managers additional paperwork is required: “Additional paperwork required and must be attached to this application, including Cover Letter, Resume, Signed Background Check and Credit Check Authorization Form, and Signed Acknowledgement of CRIT Substance Abuse Policy.”
